Approximately 90 percent of flu-related deaths each year occur in people 65 years and older. Clinicians at Wayne State University and Crittenton Hospital Medical Center developed a Lean Six Sigma program with the goal of improving immunization rates in this high-risk population. One component of the strategy was to create the fishbone diagram below to identify all the variation sources in the process that could lead to failure to immunize a patient. The team’s complete Six Sigma approach and their results were published in the Fall 2013 issue of The Ochsner Journal.
